I, the undersigned, warrant
that I am the parent or legal guardian of _______________________ (the Minor
Child), a minor child who is attending the SimuCon Convention.
I further agree to release and hold harmless Simutronics Corporation from any
responsibility for the well being, safety, and care for the Minor Child during
the SimuCon Convention. I understand that the Minor Child is my responsibility
and not the responsibility of Simutronics Corporation. If, at any time during
the SimuCon Convention, I am not in direct physical custody of the Minor
Child, I understand that I am entrusting the Minor Child to be responsible
for
themselves. I also accept full responsibility for the Minor Child's actions
while they are attending SimuCon, including but not limited to, any damages
that may result.
